1) Try to design a way the mikva lady or two could lower her into the mikva, something on the idea of a swing might work. Once she is fully in the mikva, the buoyancy of the water should allow her to stand unassisted.

2) If a woman permanently can not immerse on the mikva it is potentially problematic to remain married, a Rav who knows the details of your situation personally should be consulted.
